Ann Rheum Dis:低剂量他汀保护骨骼!奥地利35万余人研究称:高剂量却增加骨质疏松风险

近年来,有研究表明,他汀有益于骨骼健康。但近期来自奥地利的一项大规模研究表明,他汀的这种保护作用与剂量有关,即低剂量应用时可降低骨质疏松风险,高剂量应用时却增加骨质疏松风险。该研究显示,总体上,校正因糖尿病等合并症处方的其他药物以及可导致骨质疏松的糖皮质激素等用药后,与未接受他汀治疗的人相比,接受他汀治疗者出现骨质疏松的可能性增加了近3倍(OR=3.62)。但是,接受低剂量(0~10 mg/d)他

该研究显示,总体上,校正因糖尿病等合并症处方的其他药物以及可导致骨质疏松的糖皮质激素等用药后,与未接受他汀治疗的人相比,接受他汀治疗者出现骨质疏松的可能性增加了近3倍(OR=3.62)。

但是,接受低剂量(0~10 mg/d)他汀治疗者出现骨质疏松的风险降低。

不同的他汀均显示出类似的效果:低剂量应用洛伐他汀、普伐他汀、辛伐他汀、瑞舒伐他汀者骨质疏松风险分别降低61%、32%、30%、31%。

相反,以高剂量应用这些他汀时,骨质疏松风险增加:辛伐他汀超过40 mg/d、阿托伐他汀和瑞舒伐他汀超过20 mg/d时,骨质疏松风险分别增加64%、78%、104%。

研究者解释到,他汀以高剂量应用时,尽管有很强的降胆固醇效果,但同时可能会降低雌激素水平并影响骨骼,发挥类似绝经的效应,此时该作用可能明显超过了他汀的骨质保护作用(增加骨成形蛋白的表达)。

研究者建议,骨质疏松高风险人群在服用他汀的过程中应密切监测,尤其是接受高剂量他汀治疗的绝经后女士,这有助于指导该人群的个体化骨质疏松预防和治疗。

不过,来自中国香港的Ching-Lung Cheung指出,血低密度脂蛋白胆固醇水平升高本来就与骨密度降低有关,因此接受高剂量他汀治疗的患者基线骨密度就低于接受低剂量他汀治疗的患者,这样会影响到观察结果。

Cheung等人开展的研究显示,低密度脂蛋白胆固醇水平降低与股骨颈和腰椎骨密度增加有关。

另外,Cheung指出,采用国际疾病分类(ICD)编码来诊断骨质疏松也是该奥地利研究的一个重要局限性。

该研究覆盖奥地利所有90岁以下的人群(近800万人),其中35.35万人服用一种他汀至少1年,包括辛伐他汀、洛伐他汀、普伐他汀、氟伐他汀、阿托伐他汀、西立伐他汀、瑞舒伐他汀。其中,11701人诊断骨质疏松,以女士为主(9936人)。
