Stroke:糖尿病与缺血性卒中的风险增加有关

由此可见,冠状动脉造影后,不仅CAD,而且DM也与缺血性卒中的风险有关。它们的组合进一步增加了缺血性卒中的风险,具体取决于CAD的程度。

没有冠状动脉疾病(CAD)的糖尿病患者(DM)和非DM患者在冠状动脉造影后发生心肌梗塞的风险类似。DM患者中依赖于CAD状态的缺血性卒中的风险研究较少。近日,心血管疾病领域权威杂志Stroke上发表了一篇研究文章,研究人员旨在评估没有CAD的DM患者缺血性卒中的风险,是否与既没有DM也没有CAD的患者相似。

研究人员对2004年至2012年在丹麦西部进行冠状动脉造影的患者进行了队列研究。既往诊断为缺血性卒中或短暂性脑缺血发作的患者被排除在外。根据DM和CAD的存在情况对患者进行分层。冠状动脉造影后30天开始随访。研究人员以没有DM或CAD的患者为参考,计算了事件发生率并调整了发生率比值。研究人员评估了DM患者的CAD程度和缺血性卒中之间的趋势。

该研究共纳入了81909例患者。中位随访时间为3.8年。患有DM和CAD的患者发生缺血性卒中的风险最高(每100人每年1.32次事件;调整后的发生率比值为2.00[95%CI为1.72–2.32])。仅患有CAD(每100人每年0.77次事件;调整后的发生率比值为1.27[95%CI为1.12-1.44])或仅患有DM的患者(每100人每年0.95次事件;调整后的发生率比值为1.74[95%CI为1.42–2.15])处于中等风险,而既非DM又非CAD的患者(每100人每年0.52次事件)的风险最低。在DM患者中,CAD程度可进一步预测其风险(趋势P<0.001)。

由此可见,冠状动脉造影后,不仅CAD,而且DM也与缺血性卒中的风险有关。它们的组合进一步增加了缺血性卒中的风险,具体取决于CAD的程度。
