患者，女，54岁，NSCLC治疗后，为再分期行18F-FDG PET/CT检查。由于第一次检查时出现可疑肺结节，因此6个月之后再次行18F-FDG PET/CT检查。

病例简介

病史：患者，女，54岁，NSCLC治疗后，为再分期行18F-FDG PET/CT检查。由于第一次检查时出现可疑肺结节，因此6个月之后再次行18F-FDG PET/CT检查。

这名肺癌患者是否出现肺结节转移？出现了什么直肠病变？良性还是恶性？

图片说明：轴位PET、CT及融合图像（a、b、c）显示位于右肺胸膜下的微小结节，并显示FDG轻微摄取（箭）。位于直肠的肿物导致充盈缺损（箭），并可见FDG明显摄取（SUVmax 6.2）（d、e、f）。

释义：位于直肠的高代谢软组织肿物可疑为绒毛状腺瘤，肺结节考虑为转移病灶。

结果：直肠肿物切除后经病理证实为管状绒毛状腺瘤。

学习要点：口服对比剂有利于更好显示肠管的结构和肿块性病变（如绒毛状腺瘤）。良性增生性病变和癌前病变的SUVmax并没有显著差别，因此，发现高代谢病灶引起的充盈缺损时，应行结肠镜检查。
