BBC:一个来自20年慢性背痛患者的故事

很多背痛都是自愈性的,然而英国医生 Rangan Chatterjee却遇到了一个遭受20多年慢性背痛的患者,背痛给这位患者的健康和生活都带来了严重的影响。他时常因为背痛而不能工作,甚至严重时还需要住院治疗,这使他的家庭财务状况不容乐观。

25-64岁人群中,每年约有1000万个工作日因背痛而损失。80%的人在其一生中将会经历背痛。在英国,每年的损失为约120亿英镑的经济支出和巨大的个人和情感成本。

很多背痛都是自愈性的,然而英国医生 Rangan Chatterjee却遇到了一个遭受20多年慢性背痛的患者,背痛给这位患者的健康和生活都带来了严重的影响。他时常因为背痛而不能工作,甚至严重时还需要住院治疗,这使他的家庭财务状况不容乐观。

慢性背痛的原因:

排除癌症、感染和骨折等特殊病因非常重要。所以咨询医学专家就显得很有必要。值得庆幸的是,在绝大多数背痛病例中都没有发现恶性病因,这被称为非特异性背痛。对于这种背痛有各种各样可能有用的疗法。英国健康与保健研究所 (NICE) 建议这类病人尽量正常活动。

Rangan Chatterjee医生在第一次检查病人的脚时就发现左右脚不对称,并认为这是导致背痛的原因之一。接着就带病人去见了运动专家。专家从病史中了解到患者在五岁时有过右膝伤以及小腿骨折,并认为这些因素和患者的背痛有关。在评估了患者的运动模式后,专家确定患者的肌肉工作效率不足,他还评估了患者的稳定性和灵活性,发现右腿的运动范围缩小了,而左腿却存在过度使用的情况。于是为患者制定了一系列的个性化运动治疗,嘱咐患者每周训练几次。这对减轻患者的背痛起到了明显的效果。除此之外,患者还有来自失眠的压力、经济状况的焦虑以及因疾病丢掉工作的顾虑。这些压力会使大脑更容易察觉身体的疼痛。在医生的指导下患者采取了包括冥想、听音乐、打太极拳等自我调节方式。这些措施在减轻背痛上都发挥了作用。

维生素D的影响:我们发现患者的Vitamin D水平非常低,维生素D主要来源于光照下皮肤内前体物质的转化生成以及从食物中的摄入,而正常的维生素D水平对骨骼健康十分重要。患者补充Vitamin D后背痛也得到了一定程度的改善。

联合治疗方式:2016年NICE关于背部疼痛管理的指南推荐包括运动锻炼、心理治疗以及手法治疗是必要的。并且不同的病人有不同的病因,个性化治疗措施的制定是成功与否的关键。

背痛的建议:

1,设定闹钟提醒自己定期从办公桌上起身活动,也可以试着站立工作。

2,主动放松。例如瑜伽和散步。

3,找到一种自己喜欢的运动,并长期坚持。

4,尽可能积极运动,不要低估了散步的好处。
