GUT: 回肠粘膜相关菌群的组成可以预测克罗恩病术后内镜复发

回肠切除克罗恩病(CD)后,患者疾病复发非常频繁。尽管已经确定了几种复发的临床危险因素,但是预测复发仍然具有挑战性。目前医生一般建议在手术后的第一年内进行回肠结肠镜检查以评估内镜复发率并调整治疗方法,本项研究则利用一个大型的前瞻性多中心队列研究回肠粘膜相关菌群在术后内镜内镜复发中的作用。

背景及目的
回肠切除克罗恩病(CD)后,患者疾病复发非常频繁。尽管已经确定了几种复发的临床危险因素,但是预测复发仍然具有挑战性。目前医生一般建议在手术后的第一年内进行回肠结肠镜检查以评估内镜复发率并调整治疗方法,本项研究则利用一个大型的前瞻性多中心队列研究回肠粘膜相关菌群在术后内镜内镜复发中的作用。

方法
研究人员在法国招募了201例患者(共288个样本),通过手术和/或内镜评估时以及16S测序分析了回肠黏膜相关菌群。

结果
CD手术后回肠黏膜相关菌群发生了深刻变化。与非复发环境相比,内窥镜复发与回肠粘膜相关菌群的强烈变化有关,与健康受试者相比,回肠黏膜相关菌群的强烈变化非常让人联想到回肠CD中普遍观察到的变化,其中α多样性降低,部分变形杆菌增加的门类,并且在Firmicutes门内的Lachnospiraceae和Ruminococcaceae家族的一些成员减少。在手术时,研究发现了几种与内镜复发相关的细菌分类群,它们比通常的临床危险因素能更好地预测复发。

结论
手术对回肠黏膜相关菌群有重要影响。术后内镜复发与微生物群组成和α多样性的改变有关。肠道菌群有可能预测术后的发展和复发。
