NEJM:健康人群中单肾小球滤过率研究

健康的肾供体,单肾肾小球滤过率与年龄、性别和身高不相关,高单肾肾小球滤过率是慢性肾脏疾病和某些肾活检风险结果的体现。

肾小球滤过率(GFR)是肾功能评价的标准,单个肾单位肾小球滤过率是评估单个肾单位功能的标准。但单个肾单位GFR与人口学、临床特征以及肾脏活检结果的关系尚不清楚。

研究人员对1388个活体供肾者进行电脑断层扫描,使用造影剂和碘酞酸盐测量肾小球滤过率。平均单个肾单位肾小球滤过率通过肾小球滤过率除以肾单位数目得出(根据CT评估两肾的皮质体积,活检确定肾小球密度)并将人口学、临床特征及活检结果与单个肾单位肾小球滤过率相关。

研究中58%的供肾者为女性,平均年龄44±12岁。平均GFR为115±24 ml每分钟,肾单位数目平均为860000±370000每肾,平均的单肾GFR为80±40 nl每分。研究发下单肾GFR随供体年龄、性别和身高(190cm以下)变化不显著。高单肾GFR与肾活检体积,肾小球硬化和动脉硬化无关。高单肾GFR与身高超过190cm以及家族末期肾脏疾病史相关。

研究认为健康的肾供体,单肾肾小球滤过率与年龄、性别和身高不相关,高单肾肾小球滤过率是慢性肾脏疾病和某些肾活检风险结果的体现。
