Circulation:Bmpr2突变与炎症协同促进肺动脉高压的发生

骨形态发生蛋白受体2(Bmpr2)突变是遗传性肺动脉高压(PAH)的重要风险因素,约20%的携带者会罹患此病。但环境因素(如炎症)是如何驱动Bmpr2突变对PAH易感的机制尚不明确。在Bmpr2+/-小鼠中过表达5-LO(5-脂氧合酶)可引发肺部炎症和一过性PAH。因此,5-LO和它的代谢产物,白三烯B4,均是二次打击的候选物。本研究的目的是明确5-LO介导的肺部炎症如何与表型沉默的Bmpr2缺陷协同作用,诱发大鼠显著的肺血管疾病。

建立单等位基因Bmpr2突变的大鼠,观察1年确定其表型正常。为评估二次打击是否会诱发疾病,予以大鼠过表达5-LO的腺病毒、单辛他林、SU5416、SU5416伴慢性缺氧或仅慢性缺氧处理。Bmpr2突变型遗传性PAH患者的样本用于评估内膜5-LO的表达。将BMPR2信号受损的肺动脉内膜细胞暴露于增强的5-LO介导的炎症环境中,评估表型和转录本的变化。

在Bmpr2+/-大鼠中,5-LO腺病毒引起的肺部炎症,引发严重的PAH和内膜重构,但在野生型小鼠中没有相似现象。在Bmpr2+/-大鼠内膜病灶中,内源性5-LO的表达升高,与白三烯B4生物合成升高有关。Bmpr2突变型遗传性PAH患者的内膜细胞中的5-LO的表达也升高。在体外,BMPR2缺陷,加上5-LO介导的炎症,可产生具有间质特征的抗凋亡和增殖的肺动脉内皮细胞。这些转化细胞中5-LO的表达与诱导白三烯B4生产一致,转录组的特征也类似于临床疾病,包括核转录因子κB亚基(NF-κB)、白细胞介素- 6、转化生长因子β(TGF-β)信号通路上调。TGF-β拮抗剂可逆转Bmpr2突变型大鼠的PAH和血管重构,提示TGF-β对于新生内膜的转换至关重要。

在PAH的新的二次打击模型中。肺部炎症诱导Bmpr2+/-大鼠发生严重的肺动脉高压。内皮细胞转换需要激活经典和非经典的TGF-β信号通路,特征是5-LO核膜易位与白三烯B4合成增多。本研究揭示了环境损伤是如何释放其他沉默的基因突变的破坏潜能的。
