本研究旨在以关节镜或开放手术为参考标准,评价3D MRI检出膝关节软骨缺损及其影像学特点的诊断价值。

由两名研究中评价每个研究的方法学质量。利用二元随机效应模型比较3D和2D MRI检出膝关节软骨缺失的诊断比值比(DOR)并评价相关协变量对3D MRI诊断的影响。利用Meta 回归分析评价近3年3D MRI DOR的诊断价值。

结果为。本研究共纳入了27项研究,其中有16 (59%)为3D MRI与2D MRI比较检出膝关节软骨缺失的价值。近3年3D MRI诊断膝关节软骨缺失的能力具有显着性提高。3T MRI 3D序列要比1.5T及以下MRI具有更高的DOR(相对DOR, 4.05; P = .01)。与传统轴位、矢状位和冠状位2D MRI相比,三维多平面重建具有较高的特异性。

3D快速反转回波序列具有比3D MRI序列更高的敏感性和特异性(P < .05)。

本研究表明,3D MRI序列能够提供较2D MRI相似的诊断能力,并场强3.0T、3D快速反转回波序列及多平面重组具有更高的的诊断能力。
