一、第一款FDA上市的植物药:绿茶提取物Veregen 2006年10月,美国食品和药物管理局FDA批准Veregen(茶多本酚)作为新的处方药,用于局部(外部)治疗由人类乳头瘤病毒引起的生殖器疣。这是FDA根据1962年药品修正条例首个批准上市的植物(草本)药。这是一款极具中国血统的准中药,它的临床研究到成功上市离不开背后默默无闻的科学家程书钧院士。

一、第一款FDA上市的植物药:绿茶提取物Veregen

2006年10月,美国食品和药物管理局FDA批准Veregen(茶多本酚)作为新的处方药,用于局部(外部)治疗由人类乳头瘤病毒引起的生殖器疣。这是FDA根据1962年药品修正条例首个批准上市的植物(草本)药。这是一款极具中国血统的准中药,它的临床研究到成功上市离不开背后默默无闻的科学家程书钧院士。

从程书钧开始进行茶多酚的研究,到这个“里程碑”的创立已经是二十多年,到现在,大多数人最关心的话题是:茶多酚是如何赢得FDA认可的?程书钧院士说:“这是一个漫长的、复杂的国际合作的结果,但是每一步都是自然而然,我从未强求”。从开始研究绿茶儿茶素抗突变、抗癌变作用及其机理,证明绿茶儿茶素有明显的抑制皮肤炎症、增生、抑制皮肤乳头状瘤发生等作用。到后续与国外公司进行合作申请各国专利,推动各项临床试验,最终成功在多个国家或地区上市(包括美国)。这是程书钧院士在20世纪90年代研发出的治疗尖锐湿疣的组合性药物,转让给德国MediGene公司,经过多年的国际合作开发,最终在美国FDA申请了新药,这个药的发明源于他们20多年的系列实验研究和临床研究。有效性和安全性得到了诸多明确的临床数据的支持,让这款药物成功征服了FDA,其中艰辛可见一斑。

二、第一例口服植物处方药:Fulyzaq

2012年12月31日,美国FDA批准了Salix制药公司的口服植物药Crofelemer(商品名为Fulyzaq)上市,该药可用于缓解HIV/AIDS患者接受抗逆转录病毒(ART)疗法时出现的非感染性腹泻症状。这是自2006年10月FDA批准第一例植物药Veregen上市后批准的第二例植物药,Veregen是从绿茶中提取的儿茶素类和其他绿茶组分的混合物,属外用药;而Crofelemer则是FDA批准的第一例口服植物药。

Crofelemer来源于巴豆属植物lechleri的红色胶乳。当树皮被割破时,会流出一种红色类似于血样的胶乳,其中含有新型多分子结构物质Crofelemer。Crofelemer是从巴豆属植物(Croton lechleri Müll. Arg.)红色汁液中提取的植物药(botanical drug),它是原花青素(oligomeric proanthocyanidin)混合物,由(+)-catechin、(-)-epicatechin、(+)-gallocatechin、(-)-epigallocatechin四种单体随机聚合而成,平均聚合度5-7.5。

FDA发布的资料显示,Fulyzaq的安全性和有效性已得到临床研究证实。该研究在南美洲开展,共有374名HIV阳性患者参加,这些患者均接受了稳定的ART疗法,并且伴有一个月以上的腹泻(已剔除感染肠道疾病引起的腹泻病例)病史;这些患者发生水性腹泻的平均次数大概为2.5次/天。试验将患者随机分为Fulyzaq组和安慰剂组,每日给药两次。临床有效性终点的判定依据为,在至少两周的安慰剂-对照研究(为期四周)中,每周出现水性腹泻症状次数不超过两次的患者比例。研究结果表明,在服用Fulyzaq治疗的患者中,17.6%的患者有临床应答,明显高于安慰剂组8%的应答率;在部分患者中,这种抗腹泻作用甚至可持续20周之久。在2016年3月,Napo制药公司收回Fulyzaq的生产销售权(之前是由Salix Pharmaceuticals公司负责研发生产)。

研究认为,Fulyzaq作为抗分泌、抗腹泻制剂,主要作用于肠腔,具有较少的全身作用特点。目前已有研究证实,氯离子的分泌可导致腹泻的发生,并伴有脱水、电解质紊乱、腹部绞痛等症状。在推荐剂量(125mg缓释片,1片/次,2次/日)下,Fulyzaq可同时抑制cAMP激活的囊性纤维化跨膜转导调节因子(CFTR)氯离子通道和钙激活氯离子通道(CaCC),通过抑制氯离子向肠道的分泌以减少水分从循环系统向肠腔的分泌量,进而缓解患者的腹泻症状。临床研究表明,与其他抗腹泻制剂不同,Fulyzaq不会影响肠道蠕动以及患者服用HIV治疗药物的有效性和安全性。但是,在使用Fulyzaq治疗前,医护人员必须进行适当检测,以确定该腹泻并非由感染或胃肠道疾病引起的。从临床研究结果来看,服用Fulyzaq的患者中最常见的不良反应为上呼吸道感染、支气管炎、咳嗽、肠胃气胀,以及肝酶胆红素水平的升高。

三、在美国进行临床试验的国产中成药

据不完全统计,已向美国FDA递交IND申请的中成药有:

事实上,芪苈强心胶囊,在美国开展的临床研究早已完成,效果相当不错。

上图是10个在美国开展临床试验的国产中成药,根据最新数据显示,其研发进展基本未发生比较明显的进展。排在第一位的复方丹参滴丸是最受大家关注的中成药,在2017年9月,李连达院士与天士力就复方丹参滴丸美国FDA申报注册临床III期试验结果的问题刷屏医药圈,双方各执一词,好不热闹。早在1996年复方丹参滴丸被推选为第一例复方中成药申报美国FDA,到2006年天士力第二次向FDA申请新的IND,明确以预防和治疗慢性稳定性心绞痛作为适应症,这是目前为止中国中药国际化进展最快的品种。其能否最终在美国上市,是关系到我国中药国际化的里程碑式的大事件。然而2018年即将过去,相关的定论却未能盖棺,国产中成药能否开创历史,得到美国FDA的认可,仍需要时间去验证。

四、两款植物药在FDA获批的启示

FDA对含多成分的植物药的审批非常谨慎,植物药产品的生产商必须确保对原材料的严格质量控制,执行种植和采收的生产规范以及复杂混合物的分析测试。因此我国植物药(如临床阶段的HMPL-004)想要进入国际市场并非易事,从中药材到成品的整个过程需进行严格的质量控制,同时注重安全性和有效性研究,结合国际植物药研究经验,选择合适的适应症和剂型进行开发。

绿茶提取物的功效最初由我国学者程书钧院士发现,并以论文形式在国际期刊发表。然而,该产品的后期研发工作却由日本、德国等多方完成,并最终在美国上市,成为FDA批准的第一例植物药,其他如紫杉醇等也是如此。忽略基础研究与产业化联系的结果,便是为外人苦做嫁衣。国内企业应重视产学研一体化作用,在深入开展产品研究的基础上,夯实品种的产业化推广工作,做到充分地用试验数据说话,靠明确的有效性和安全性去征服审评审批的评委,而不是靠情怀、哲学或者文化去发展中医药。愿中医药能在吾辈手中发扬光大。
