针对急性大咯血，多数研究认为即使患者适合行急症肺部切除术，支气管动脉栓塞仍为初始治的首选。近日来自南非德班市Inkosi Albert Luthuli中心医院的Gerard R. Alexander进行了一项回顾性分析研究，该研究发现针对急性大咯血的患者可以直接行急症手术。该研究结果发表在2013年8月4日的欧洲胸心血管外科杂志上（European Journal of Cardio-Thoracic Surgery）。

2005年1月-2007年10月于德班市Inkosi Albert Luthuli中心医院胸心外科就诊的适合行急症肺部切除术的大咯血患者被纳入研究组。根据急症临床评估，判断急诊肺部切除术前是否行BAE.如果患者行BAE，依据大咯血复发时间行肺部切除术。适合行急症肺部切除术的大咯血患者共61例。41例未行BAE直接行急症肺部切除术，1例术后复发轻度咯血。余并发症包括2例死亡，1例术后脓胸，1例胸部切口重度感染。20例术前行BAE，15例患者（75%）行BAE后复发咯血。术后无复发咯血。余并发症包括1例死亡，2例术后脓胸。

该回顾性研究显示：针对影像学资料显示病灶局限的大咯血患者可直接行急诊肺部切除术。数据显示针对不适合行急症肺部切除术的患者，BAE是一种有效的缓解性治疗措施。
