生长分化因子15 (Growth differentiation factor 15, GDF-15)是一种应激反应性生物标志物，目前研究表明， GDF-15在不同的疾病环境中与死亡率和心血管事件独立相关，但关于其在卒中后预后价值的数据仅有少数报道。

在此，研究人员对3066例急性缺血性脑卒中患者进行血清GDF-15基线测定。主要结果是3个月内死亡和严重残疾。次要结果包括死亡、主要残疾、血管事件和卒中复发。采用多因素logistic回归或Cox比例危险模型评估GDF-15与卒中后临床结局的关系。

随访3个月，676例(22.05%)、86例(2.80%)、81例(2.64%)、51例(1.66%)患者分别出现严重残疾、死亡、血管事件或卒中复发。在校正年龄、性别、吸烟、饮酒、和国立卫生研究院的中风尺度基线分数，1 SD八进制数数对数转换GDF-15主要结果比值比/风险比(95% CI) 为1.26(1.15 - -1.39)，重大残疾比值比/风险比为1.13(1.02 - -1.25)，死亡1.79(1.48 - -2.16)，血管事件比值比/风险比为1.26(1.00 - -1.58)。将GDF-15加入已建立的危险因素中，可提高死亡与重大残疾综合结局的风险预测(c统计量、净重分类指数、综合判别改善均P < 0.05)。

研究表明，GDF-15浓度较高与急性缺血性脑卒中的不良临床结局独立相关，提示基线血清GDF-15可为鉴别预后不良高危缺血性脑卒中患者提供额外信息。
