对于糖尿病患者，如果收缩压在130~140mmHg，服不服降压药？

2017年美国心脏病学会/美国心脏协会（ACC/AHA）高血压指南推荐，糖尿病高血压患者启动降压药的门槛是130 mmHg。

但南方医科大学Wang S等进行的Meta分析显示，收缩压在130~140 mmHg的糖尿病患者使用降压药物治疗，可能得不偿失，不仅心血管和肾脏无获益，而且不良事件风险还增加。

研究显示，收缩压在130~140 mmHg的糖尿病患者启动降压，并未降低主要心血管事件的发生率（RR=1.01，95%CI：0.93~1.10），而且在亚组分析和敏感性分析中是一致的。

此外，降压治疗也没有降低心肌梗死风险、脑卒中风险、心衰风险以及白蛋白尿风险、终末期肾病风险、心血管死亡和全因死亡风险。



除了无任何获益外，还可能增加严重不良事件的风险（RR=2.00，95%CI：1.33~3.01）和低血压风险（RR=5.30，95%CI：0.99~28.40）。

研究者指出，这提示可能不建议糖尿病患者收缩压在低于140 mmHg时开始降压。

对于2017年美国新指南，其中多采纳SPRINT研究的事后分析，但其中纳入的并非糖尿病患者，而是糖尿病前期患者，二者有所差异。

这项研究共纳入6项试验21574例糖尿病患者。
