肥胖与CKD的发生发展有关，但减肥手术是否能对CKD发挥保护性作用尚不清楚。近日，肾脏病领域权威杂志Journal of the American Society of Nephrology上发表一篇研究文章，研究人员旨在评估减肥手术是否可以影响CKD风险。
 
这项研究纳入了2144名在2006年3月至2009年4月期间接受减肥手术的成年受试者，并且参与了减肥手术纵向评价2队列研究。该研究的主要结局为通过结合eGFR和蛋白尿根据改善全球肾脏病预后组织(KDIGO)的协会标准确定的CKD风险分类。
 
该研究的患者79%为女性，87%为白人，年龄中位数为46岁。在中等基础CKD风险（分别为63%和53%）、高基础CKD风险（分别为78%和56%）和极高基础CKD风险（分别为59%和23%）的患者中减肥手术后1和7年可以观察到CKD风险改善。CKD风险恶化的患者比例为≤10%；五例患者发生终末期肾脏疾病。以1年为基线的敏感性分析，以尽量减少体重对血清肌酐和不同的eGFR方程的影响，得到了相似的定性结果。减肥手术治疗与CKD风险类别改善在很大比例的长达7年的患者中相关性较为显著，特别是在那些中度和高基础CKD风险中。

由此可见，这些发现支持考虑减肥手术时评估CKD风险，并进一步研究减肥手术作为高危肥胖CKD患者的一种治疗手段。
