CLIN CANCER RES近期发表了一篇文章，研究阿片类镇痛药通过调节μ-阿片受体（MOR），Toll样受体4（TLR4）及基质降解等对肿瘤转移产生潜在的影响。

作者收集了60例患者术前及术后3，6，和24小时的血液标本，在相同的时间点进行疼痛评分。记录阿片类药物使用情况。作者还收集了10例健康志愿者的血液标本。使用Alphascreen AMP分析和MOR过表达细胞评估MOR活化情况。使用HEK-Blue hTLR4评估TLR4活化情况。使用明胶酶谱法评估循环基质金属蛋白酶活性。研究结果表明，术后血浆样本可以活化MOR并抑制LPS诱导的TLR4活性。线性模型分析表明MOR活化可以显着抑制LPS诱导的TLR4活化。此外，TLR4在理解疼痛评分时具有重要作用。术后血液样本也表现出循环基质降解酶活性改变，但这一改变与阿片类药物使用及MOR活化均无关。

文章最后认为，该研究第一次发现手术患者阿片类药物使用会调节TLR4活性且术后疼痛与循环TLR4活化增加有关。这些结果支持使用MOR活化而不是阿片类摄入评估特定时间点的阿片类暴露情况。

采用PECS阻滞及其修饰术在全麻乳腺手术术中及术后第一个24小时内均具有显著的阿片类药物节约效应。与PVB组相比，它的术中阿片类药物节约效应更高。在术后第1天，PVB组的吗啡节约效应更高，但并发症的发生率也更高。目前的PECS阻滞技术显示出明显的研究间变化并需要标准化。
