Cancer Cell:多组学分析助力寻找恶性脑胶质瘤的代谢“破绽”

弥漫性先天性脑桥神经胶质瘤(DIPG)是一种高度侵袭性儿童脑干肿瘤,其中80%以上患者存在组蛋白H3K27位点杂合性点突变(H3K27M),并会导致总的H3K27me3水平的降低。

弥漫性先天性脑桥神经胶质瘤(DIPG)是一种高度侵袭性儿童脑干肿瘤,其中80%以上患者存在组蛋白H3K27位点杂合性点突变(H3K27M),并会导致总的H3K27me3水平的降低。由于DIPG无法通过手术切除,且常规的放化疗手段对其几乎没有作用,因此迫切需要找到一种有效的DIPG治疗方法。目前,针对携带H3K27M突变DIPG的一个主要的治疗思路就是通过表观的方法提升细胞内H3K27me3水平来抑制肿瘤的生长。

2020年8月13日,来自美国密歇根大学Sriram Venneti教授研究团队在Cancer Cell上发表了题为Integrated Metabolic and Epigenomic Reprograming by H3K27M Mutations in Diffuse Intrinsic Pontine Gliomas的文章,报道了携带H3K27M突变的 弥漫性先天性脑桥神经胶质瘤(DIPG)通过代谢重编程使细胞内ɑ-KG始终维持在一个较高的水平,进而激活以ɑ-KG为辅因子的双加氧酶,降低细胞内H3K27me3水平。基于这一代谢特征,作者提出了通过靶向ɑ-KG的生成来杀死H3K27M DIPG的新策略。

首先,为了探究H3K27M DIPG的代谢特点,作者整合了转录组、蛋白质组和代谢组对H3.3WT和H3K27M突变的小鼠神经元干细胞(NSCs)进行多组学的分析比较,作者发现H3K27M突变的NSCs的糖酵解与TCA循环的水平是增加的。接下来,通过对DIPG患者来源的肿瘤样本进行单细胞测序(sc RNA-seq)和磁共振波谱分析(MRS),作者再次得到了这一结论。

其中,代谢组学的结果显示:含H3K27M突变的细胞中α-KG的水平与WT细胞相比有着显着的升高,而α-KG作为一系列去甲基化酶的辅因子(例如,TET,KDMs),对于细胞内组蛋白的甲基化水平具有重要的调控作用。因此,作者推测H3K27M细胞中升高的α-KG可能是导致细胞内H3K27me3水平下降的重要原因。紧接着,作者在细胞层面对这一猜想展开了验证。作者发现:对携带H3K27M突变的细胞进行Glutamine和Glucose饥饿处理可以显着升高H3K27me3的水平,并会大幅抑制细胞的增殖,而WT的细胞影响则相对较小,同时这一表型在给细胞回补了酯化的α-KG之后得到了恢复。进一步,抑制Glutamine和Glucose生成α-KG通路中的代谢酶如GDH,HK2,IDH1/2,也观察到了相同的实验现象。这些实验结果表明,阻断由Glutamine 和Glucose生成α-KG可以提高含H3K27M突变细胞的H3K27me3水平,进而抑制携带这一突变的DIPG的生长。

最后,考虑到H3K27M突变DIPG的这一代谢特征具有十分重要的临床应用前景,作者在动物水平展开了进一步的验证。作者选取了两种可以穿过血脑屏障的小分子抑制剂,JHU-083(glutamine结构类似物)以及一种IDH的共价结合抑制剂。作者发现这两种抑制剂均能在小鼠体内有效抑制H3K27M DIPG的生长,且两者的联合使用效果则更加显着。

综上所述,该研究团队利用多组学的研究手段鉴定出携带H3K27M突变的 DIPG一个重要的代谢特征,即依赖于糖酵解和TCA循环来维持高α-KG pool,进而使H3K27位点处于低甲基化水平。因此,该发现为这类DIPG提供了新的治疗靶点。

这里值得一提的是,之前的研究报道携带IDH1/2突变的脑胶质瘤会大量产生α-KG的结构类似物D-2HG,进而会竞争性地抑制去细胞内甲基化酶的活性增加组蛋白的甲基化水平,这一看似矛盾的现象说明这两种胶质瘤细胞会采取相反的方式来调控α-KG的水平进而维持自身最适宜的表观状态,因此这两种突变对于胶质瘤细胞而言存在很强的专一性。同时,作者的实验结果也证明:人为的将这两种突变组合在一起会使细胞存在“合成致死”效应,这一发现或许为这两种脑胶质瘤的治疗提供了新的思路。
