Clin Chem:循环游离DNA下一代测序NGS在评估转移性乳腺癌突变与基因扩增中的应用

乳腺癌组织具有高度异型性,表现为大量体细胞突变和体细胞拷贝数改变(CDAs)。该研究用一种新型靶向下一代测序盘(NGS)对游离DNA(cfDNA)进行测序,检测转移性乳腺癌(MBC)女性患者的体细胞突变与基因扩增。

用一个含有158个扩增子包含16个基因热点突变和拷贝数目改变的自定义面板,从42名确证的未治疗的转移性乳腺癌患者与9名健康对照组中提取游离DNA(cfDNA)样本,每一个cfDNA样本都与配对淋巴细胞DNA通过NGS进行比较,用液滴数字PCR对比较结果进行进一步验证。

健康对照组中的cfDNA样本没有发现存在突变,然而,1/2转移性乳腺癌患者的cfDNA样本中13个基因里检测到至少一个基因存在突变和扩增(突变数均值为2,范围为1-6).纵向随访研究发现,cfDNA中基因突变与扩增的动态变化比肿瘤抗原C15-3更能动态展现治疗过程中产生的克隆性或亚克隆性反应。值得注意的是,发生进展性血液疾病的7名cfDNA中存在ERB-B2受体络氨酸酶2(ERBB2)基因扩增的患者中,其中3名患者的内皮生长因子受体2在最初疾病诊断时表现为阴性,这说明治疗过程中突变克隆演进成更具有侵袭性的显型。最后,6名患者的cfDNA样本中找到雌激素受体1(ESR1)突变,该突变意味着其对内分泌疗法产生耐药性抵抗。综上,42名患者中,其中9名(21%)在治疗过程中,其cfDNA发生了改变。

cfDNA的靶向性下一代测序技术(NGS)有望通过检测基因突变或基因扩增的变化,监测靶向治疗效果,分析进展性肿瘤的异质性,对进展性肿瘤的靶向治疗进行分层。
