Radiology:光声成像鉴别良恶性乳腺病变的价值

本研究共纳入了2105例女性患者,由7名阅片者独立的利用OA/US与US评价乳腺肿块的良恶性,并与BI-RADS分级进行比较。纳入标准为BI-RADS3/4/5级肿块且进行了诊断性US检查及组织病理学检查;BI-RADS 3级肿块12月内性质无变化。阅片者独立评价OA/US上的US图像,并做出恶性可能性(POM)清晰和BI-RADS分级和最终诊断。同一阅片者然后分析OA/US图像,对OA特征进行评分,做出OA/US POM和BI-RADS分级。计算US和OA/US的敏感性和特异性。比较良恶性肿块的上调比率、下调比率、阳性和阴性预测值、阳性和阴性似然比。

在2105例患者中共有2191处病变,有100例患者(103处病变)作为练习组并未纳入分析。由于检查原因另外202例患者被排除,由于方案偏倚72例(78处病变)患者被排除,由于高风险组织学结果41例(43处病变)患者被排除。在1690例患者(1757处病变,良性为1079 [61.4%]处、恶性为678 [38.6%] 处),OA/US下调40.8%(3078/7535)良性病变,特异性(3242/7538, 99% CI: 40.4%, 45.7%),其中US的特异性为28.1% (2120/7543, 99% CI: 25.8%, 30.5%)。OA/US特异性超出US14.9% (P < .0001; 99% CI: 12.9, 16.9%)。对于活检为恶性的病变,OA/US和US的敏感性分别为96.0% (4553/4745, 99% CI: 94.5%, 97.0%)、98.6% (4680/4746, 99% CI: 97.8%, 99.1%) (P < .0001)。OA/US的阴性似然比为0.094,这表明检查阴性能够将US结果17.8% (low BI-RADS 4B)减少到2% (BI-RADS 3)。

本研究表明,与灰阶US检查比较,OA/US增加了鉴别良恶性乳腺病变的特异性。
