Blood:抗BMP6抗体联合EPO治疗慢性病性贫血!

联合抗BMP6抗体和EPO治疗可改善全身铁供应,减少治疗ACD所需要的EPO量。 FPN水平对红细胞前体的调节导致细胞内游离铁水平降低,并促进红细胞成熟。

重组促红细胞生成素(EPO)和铁代谢物是治疗包括慢性肾病贫血在内的与慢性炎症相关的贫血的标准方案。一个警示EPO疗法并关注与高剂量铁补充剂相关副作用,以及随着时间的推移对EPO产生耐药性的黑盒子,解释了医学需要制定新的策略来改善慢性疾病贫血(ACD)。

由于铁调素是ACD铁限制表型的核心,近期研究人员开发了一种新的针对铁调素的治疗方法。Petzer等人在此报告了一种全人抗BMP6抗体(KY1070)作为单一疗法或与Darbepoetin alfa联合用于对两种不同的、已建立的和临床相关的ACD啮齿动物模型的铁代谢和贫血消退的治疗效果。

除了抵消肝调素驱动的红细胞生成的铁限制外,研究人员还发现与单一疗法相比,KY1070和重组人EPO联合治疗可在定性和定量方面改善红细胞反应。因此,KY1070和Darbepoetin alfa联用带来了一种节省EPO的效益。

此外,研究人员还发现,通过KY1070抑制铁调素可以调节红细胞前体细胞的运铁素表达,从而降低细胞内潜在的无毒游离铁水平,并加速红细胞输出,这提示红细胞前体细胞成熟增多。

总而言之,本研究表明ACD是一种高度复杂的疾病,通过动员内源性铁沉积和刺激红细胞生成的多因素疗法,可使ACD的治疗变得更加有效。
