BMC Cardiovasc Disord：低三碘甲状腺素综合征和硒缺乏症-晚期心力衰竭患者的价值低估

脱碘酶-转换甲状腺激素的硒蛋白的功能可能会因心力衰竭伴随的氧化应激而受到干扰。谷胱甘肽过氧化物酶可能会使用硒（Se），导致缺乏脱碘酶和三碘甲状腺素（T3）。本研究旨在评估低T3综合征在心力衰竭中的患病率和临床意义，以及评估低fT3和硒缺乏症的相关性。

 

该研究组由59例因HFrEF NYHA III或IV代偿失调住院的连续患者组成。排除标准为：甲状腺功能障碍，严重的全身疾病，胺碘酮，类固醇或普萘洛尔治疗。 A组包括9例游离T3（fT3）浓度低于3.1µpmol /L的患者。B组由其余50名fT3水平正常的患者组成。

 

结果显示，低T3综合征的患病率为15.3％。硒缺乏症的患病率为74.6％。我们证明了fT3与主要临床变量（即NT-proBNP，LVEF，hsCRP）之间的相关性，但未发现fT3与Se水平之间的相关性。Kaplan-Meier生存分析表明，fT3低的患者生存率较低（p <0.001）。

 

总之，该研究结果表明，HFrEF患者经常发现低T3综合征，并伴有不良预后。我们没有发现Se和fT3水平之间的任何显著相关性。
