Blood:通过13C-MRI检测致病性T细胞的糖酵解代谢或可早早发现GvHD

糖酵解增强是GvHD早期致病性T细胞的主要代谢特征。代谢MRI使用糖酵解示踪剂可在动物出现GvHD表型特征之前检测出目标器官中的GvHD。

移植物抗宿主病 (GvHD)是异基因造血干细胞移植(AHSCT)的主要障碍。明确诊断移植物抗宿主病需要侵袭性的检查方法,对受检组织进行活检,但会有较高的出血和感染风险。

T细胞是GvHD发病机制的核心,Assmann等既往在慢性GvHD小鼠模型中的研究表明,在明显症状出现之前,同种异体反应的CD4+ T细胞就已经进入了靶器官。

鉴于糖酵解增加是T细胞活化的早期特征,Assmann等推测体内糖酵解代谢成像可无创的检测肝脏GvHD,因为GvHD发生时会有激活CD4+T细胞进入。

经实验验证,超极化的13C-丙酮酸MRI可在动物出现症状之前就检测到肝脏中丙酮酸-乳酸转化率增加,但在随后有明显症状的cGvHD期间则没有此现象。

同时,通过转录组、蛋白、代谢物和体外代谢活性分析证实了CD4+ T效应记忆细胞(主要的致病性CD4+ T细胞亚群)具有高糖酵解作用。对接受异基因HSCT治疗的患者循环T细胞的单细胞测序的初步数据也提示糖酵解增加可能是早期GvHD的一个特征。

综上所述,代谢成像在临床的应用越来越多,并或可有助于AHSCT后的无创的早期发现GvHD。
