历时3年,在近千人的大型临床队列研究中发现,白天经常犯困、夜间睡眠不足或睡眠过多均可增加认知障碍发生风险,而每晚睡6个小时~7个小时可降低认知障碍发生风险,每晚10点入睡效果最好。

近日,复旦大学附属华山医院神经内科郁金泰教授携手青岛大学青岛市立医院神经内科谭兰教授团队,历时3年,在近千人的大型临床队列研究中发现:

白天经常犯困、夜间睡眠不足或睡眠过多均可增加认知障碍发生风险,而每晚睡6个小时~7个小时可降低认知障碍发生风险,每晚10点入睡效果最好。

如果夜间睡眠时间少于4个小时或超过10个小时,认知障碍发生风险将会显着升高。

相关研究结果近日以:Sleep characteristics and cerebrospinal fluid biomarkers of Alzheimer’s disease pathology in cognitively intact older adults: The CABLE study 为题发表在 Alzheimer's & Dementia 杂志上。

据郁金泰教授介绍,他们首先建立了大规模的认知障碍临床研究数据库和生物样本库,在建立的CABLE队列(中国人阿尔茨海默病生物标记和生活方式研究)的基础上,深入探讨了各种睡眠特征对阿尔茨海默病病理生理变化的影响。

研究团队首次从生物标志物层面证实了在中老年人群中,夜间睡眠时间与阿尔茨海默病病理发生之间的U型关系:睡眠不足或过多均可促进体内淀粉样蛋白异常沉积,而淀粉样蛋白沉积是诱发阿尔茨海默病的关键因素,也是阿尔茨海默病核心病理特征之一。

日间功能障碍(如白天经常犯困)亦可促进体内淀粉样蛋白异常沉积。而最佳的睡眠模式为,每晚10点入睡,睡眠时长6个小时~7个小时,这种睡眠模式下脑内淀粉样蛋白异常沉积水平最低。

郁金泰说,他们团队还绘制了睡眠与认知障碍(包括阿尔茨海默病)风险之间关联的证据体系,结果发现10种睡眠特征(障碍)可促进认知障碍发生,其中6种有中等强度以上证据支撑,包括失眠、睡眠分裂、日间功能障碍等,以及在床上时间过多。

进一步剂量反应分析发现,夜间睡眠时间与阿尔茨海默病风险呈U型关系:即最佳夜间睡眠时长可能介于5.6个小时到7个小时之间;夜间睡眠时间少于4个小时或超过10个小时,认知障碍发生风险将会显着升高。
