JAMA Intern Med:CT辐射剂量反馈用于减少不必要电离辐射

研究认为,CT辐射剂量反馈是降低不必要辐射的有效手段

CT辐射剂量存在较大的机构间差异,且剂量通常高于需要。近日研究人员评了估干预措施对减少 CT患者辐射剂量的有效性。
 
研究在6个国家的100放射科室开展,864080名18岁以上的成年人进行腹部、胸部、腹部和胸部联合或头部的CT检查。收集CT设备的辐射剂量指标,并与其他设备进行比较,针对反馈结果,进行有针对性的建议、为期7周的改进协作和最佳做法共享。研究的主要终点是高剂量CT扫描的比例和平均有效剂量。次要终点是器官剂量,并将干预前后结果进行比较。
 
864080名成人接受了1156657次CT扫描。干预显著降低了高剂量CT扫描的比例,高剂量扫描比例的绝对变化为1.1%-7.9%,高剂量扫描比例下降4%至30% (腹部:优势比[OR]:0.82;胸部:OR:0.92;腹部和胸部联合:OR:0.49;头部:OR:0.71)。当使用器官剂量测量时,高剂量扫描的比例降低更大。高剂量扫描的绝对减少了6.0% -17.2% ,反映了整个解剖区域的高剂量扫描比例减少了23%-58%。腹部(减少6%)、胸部(4%)、胸部和腹部联合(14%)CT扫描干预后,平均有效剂量显著降低。在解剖区域,平均器官剂量的大幅度减少为8%-43%。放射科医生对CT图像质量的满意度在研究时期都没有变化,均保持较高水平。
