研究者纳入921例心绞痛但冠脉没有狭窄的连续患者(男性362例,平均年龄62±12岁),血管造影后直接实施冠脉内乙酰胆碱激发试验。结果为,心外膜痉挛总发生率为33.4%,微血管痉挛总发生率为24.2%。。心外膜痉挛往往为弥漫性,位于远端冠脉节段(P<0.01)。没有出现致死性或不可逆的非致死性并发症,但是9例(1%)患者发生了轻度并发症[非持续性室性心动过速(n=1),快速阵发性心房颤动(n=1),有症状的心动过缓(n=6),心导管诱发的痉挛(n=1)]。

美国布里格姆和妇女医院金利(Scott Kinlay)教授在同期述评中指出,尽管冠脉内乙酰胆碱激发试验的风险很低,但是在左主干病变、多支病变、重度左室功能障碍或早期心力衰竭患者中可能风险较高。在临床实践中,是否需要常规进行冠脉内乙酰胆碱激发试验仍不确定,因为其不太可能改变大多数冠心病患者的临床实践。冠脉内乙酰胆碱激发试验主要对于冠脉没有狭窄、症状顽固或不明原因心源性猝死患者中价值较大,而在左主干病变、多支病变、重度左室功能障碍或早期心力衰竭患者中应慎用。
