目前尚不确定多种细胞因子与糖尿病进展之间的长期关联情况。近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,研究人员旨在确定在10年内哪些细胞因子可以预测糖尿病前期和2型糖尿病的进展。

该研究纳入了来自Ansung队列的912名年龄在40至69岁的参与者,是韩国基因组流行病学研究的一部分。在基线时,受试者进行了75 g口服葡萄糖耐量试验和八种细胞因子的测定:纤溶酶原激活物抑制剂1(PAI-1)、抵抗素、白介素6、瘦素、单核细胞趋化因子1、肿瘤坏死因子 -α、视黄醇结合蛋白4(RBP4)和脂联素。糖耐量正常(NGT,n=241)和糖尿病前期(n=330)的受试者每两年进行一次随访,为期10年。研究人员使用多重Logistic回归分析来评估新发糖尿病和2型糖尿病患者细胞因子的预测价值。

在10年时,分别有38(15.8%)和82(34.0%)名NGT受试者已转变为糖尿病前期和2型糖尿病患者。在糖尿病前期患者中,有228人(69.1%)已转变为2型糖尿病。在基线时NGT或糖尿病前期受试者中,RBP4的最高三分位数分别与2型糖尿病的风险增加5.48倍和2.43倍相关。PAI-1最高三分位数和脂联素最低三分位数的受试者从NGT转换为糖尿病前期的几率分别为3.23和3.37。在基线糖尿病前期患者中,抵抗素最高三分位数的人群罹患2型糖尿病的可能性增加2.94倍(所有p<0.05)。

由此可见,在这项为期10年的前瞻性研究中,NGT的血清RBP4和PAI-1较高,并且脂联素含量较低与新发糖尿病和2型糖尿病有关。
