NCCN指南将奥希替尼作为EGFR敏感突变晚期NSCLC一线治疗首选推荐的依据

目前第一代、第二代以及第三代EGFR TKI均已成功上市。2019年NCCN指南将第三代EGFR TKI奥希替尼列入EGFR敏感突变晚期NSCLC的一线治疗首选推荐。从临床医生角度来看,晚期EGFR敏感突变NSCLC的治疗选择至关重要,因为这极大地关系到患者的生存获益。FLAURA研究结果显示,一线使用奥希替尼治疗可以使EGFR敏感突变晚期NSCLC患者获得18.9个月的无进展生存(PFS)获益,而传统第一代EGFR TKI带来的PFS仅为10.2个月。

肺癌脑转移防治方面,随着EGFR TKI治疗的普及,肺癌患者的生存期得以明显改善,对脑转移的关注度也随之增加,如何控制好脑转移成为临床医生必须要面临的一个严峻问题。奥希替尼的出现给脑转移患者带来了新的治疗选择,AURA系列研究表明与传统治疗方案相比,奥希替尼可以显着改善脑转移患者的PFS。

在安全性方面,FLAURA研究结果显示,相比于第一代EGFR TKI,奥希替尼不良反应发生率更低,安全性更好。基于上述三点优势,NCCN指南将奥希替尼列入到EGFR敏感突变晚期NSCLC的一线治疗首选推荐。

EGFR敏感突变晚期NSCLC的TKI治疗策略

目前临床对于EGFR敏感突变晚期NSCLC治疗是选择单药亦或是序贯治疗,尚未达成共识。基于FLAURA研究公布的结果,一线奥希替尼治疗可以给患者带来明显PFS获益,在脑转移患者中也同样显示出明显疗效和安全性。在临床工作中,如果选择从第一代EGFR TKI序贯治疗,在第一代EGFR TKI耐药以后T790M突变发生率在50%左右,很多患者会丧失接受奥希替尼治疗的机会,而将奥希替尼作为一线治疗推荐则会避免这种问题,这也是临床医生需要考虑的重要问题之一。

奥希替尼一线治疗优先推荐人群

对于追求生活质量、对药物安全性要求高的患者,奥希替尼可以作为一线治疗的优先选择。奥希替尼对于发生脑转移的患者有明显治疗优势,因此优先推荐一线使用奥希替尼治疗。
