长链n-3多不饱和脂肪酸与缺血性卒中及其主要亚型之间的相关性尚未确定。近日,卒中领域权威杂志Stroke上发表了一篇研究文章,研究人员的目的是前瞻性的探讨循环中二十碳五烯酸、二十二碳五烯酸(DPA)和二十二碳六烯酸(DHA)与总缺血、动脉粥样硬化和心源性卒中风险之间的相关性。

研究人员测定了美国3个独立的队列:CHS(心血管健康研究)、NHS(护士健康研究)和HPFS(卫生专业人员随访研究)受试者循环中磷脂脂肪酸的基线水平。缺血性中风患者根据通过影像学和医疗记录被分为动脉粥样硬化性(大、小血管梗死)或心源性。根据脂肪酸水平的风险是通过Cox比例风险(CHS)或条件Logistic回归(NHS,HPFS)来进行评估。队列研究结果使用了固定效应荟萃分析进行汇总。

平均随访11.2年(CHS)和8.3年(NHS和HPFS合并)后,共有953人发生了缺血性卒中(动脉粥样硬化性有408人、256人为心源性和289人未确定亚型)。多变量调整后,在DPA(最高与最低四分位数;合并的风险比[HR]为0.74;95%可信区间[CI]为0.58-0.92)和DHA(HR为0.80;95%CI为0.64-1)较高的个体,总的缺血性卒中的风险降低,但在二十碳五烯酸中并不降低(HR为0.94;95%CI为0.77-1.19)。DHA是降低的动脉粥样硬化性卒中风险相关(HR为0.53;95%CI为0.34-0.83),而DPA与降低的心源性脑卒中风险相关(HR为0.58;95%CI为0.37-0.92)。汇总的结果与每个队列研究的结果相一致。

由此可见,循环中高水平DHA与动脉粥样硬化性脑卒中以及DPA与心源性脑卒中的发生呈负相关。这些新的发现表明DHA、DPA和二十碳五烯酸产生有益效应的作用途径不一样。
