鉴于缺乏包括两臂比较的随机效应剂量反应统计模型,目前在临床试验中,还是很难评估剂量反应关系的确切形状。

Tommaso Filippini等充分检索了研究改变膳食钠摄入量对BP的影响的文献后,采用新的1级三次样条混合效应模型进行了剂量反应荟萃分析。纳入的试验标准:至少随访了4周、有24小时尿钠排泄检测数据、通过饮食变化和/或补充钠来调整钠的摄入量、有干预前后的收缩压和舒张压数据。

研究流程

Tommaso Filippini等共确定了85项符合要求的试验,钠摄入量浮动在0.4-7.6 g/d,随访时间为4-36个月。这些试验的受试者有高血压患者(n=65)、非高血压患者(n=11)或二者都有(n=9)。

钠摄入量与血压的剂量反应关系

总体上,汇总的数据与钠摄入量、平均收缩压和舒张压之间的近似线性关系兼容,在最低或最高钠暴露水平下均无曲线变平的迹象。在有无高血压的受试者中的结果相似,但高血压患者钠摄入减少后血压急剧下降。干预持续时间(>12周 vs 4-11周)、研究设计类型(平行 vs 交叉)、有无采用抗高血压药物和受试者的性别对钠摄入量减少对BP的影响效应均有微弱的影响。

总而言之,本次对临床适应钠摄入量减少的剂量反应分析表明,在饮食钠摄入量的整体范围内,钠摄入量与收缩压和舒张压降低之间存在近似线性关系。虽然这种相关性与基线血压高低无关,但在血压水平较高的受试者中,钠摄入量减少对血压水平的影响更为明显。
