维生素D免疫系统和炎症细胞因子的调节中起着重要作用。冠状动脉搭桥术(CABG)和心肺分流术(CPB)与炎症反应有关。近日,一项新的研究探讨了维生素D治疗对CABG后形成的细胞凋亡和炎症变化的影响,研究结果已发表于J Cardiovasc Pharmacol Ther。

 

研究共纳入在70名接受CABG手术的CPB患者。这些患者在手术前随机给予安慰剂或每日口服15万IU维生素D,连续服用3天。取右心房样本测量caspase 2、3和7的活性。每隔一段时间比较白细胞介素-10(IL-10)和胰岛素样生长因子1(IGF-1)的血清水平。

 

结果显示,维生素D组的caspase 2和3的平均阳性细胞数较少(分别为P = 0.006和P <0.001)。术前接受维生素D治疗3天后,血清中IL-10水平有所上升(维生素D组=4.4±4.9 ng/mL,对照组=1±0.5 ng/mL,P = .001)。术后,两组IL-10的水平均升高,维生素D组的水平更高(P<.001)。3天后组间血清IGF-1的水平显著差异(P = .006),CPB后维生素D组的水仍较高(P < .001)。

 

综上所述,这些研究结果表明,维生素D可以降低CPB后的细胞凋亡率,补充维生素D治疗可改善患者手术前后的炎症状态。仍需进一步的研究探究维生素D的抗凋亡特性及临床意义。
