Radiology:平扫CT对急性脑梗死的检测:通过机器学习缩小与MRI的差距

由于受累大脑区域的密度和纹理变化非常细微,并可能因正常的生理变化或陈旧病变而混淆,因此根据平扫CT对梗死灶进行定量评估极具有挑战性。由于脑组织图像的信噪比、对比度较低以及层厚较厚,使得大多数传统的基于图像的分割方法变得困难重重。现阶段,一项相研究探索了将深度学习技术(卷积神经网络结构)应用于这一具有挑战性的问题。然而,该项工作只能用于定性地检测脑半球水平缺血性病变的存在与否。

近日,发表在Radiology杂志的一项研究以扩散加权(DW) MRI作为参考标准,建立了一种利用平扫CT图像自动检测和定量AIS患者脑梗死的方法,为临床早期识别及诊断脑梗死病灶并制定最佳的治疗方案提供了可靠的技术支持。

本研究回顾性地对2004年5月至2009年7月期间AIS发生后1小时内进行弥散加权(DW) MRI检查的AIS患者(从症状出现到CT时间<6小时)的平扫CT图像进行了评估。以DW MRI图像上人工勾画的缺血性病变为参考标准。提出了一种基于机器学习(ML)的梗死病灶自动分割方法。从157例患者的平扫CT图像中随机选取,并在DW MRI图像上手动勾画病变标记以训练和验证ML模型;其余100例独立于来源队列的患者用于模型的检测。采用Bland-Altman图和Pearson相关性对ML算法与参考标准(DW MRI)进行定量比较。

在测试数据集中的100例患者中(中位年龄,69岁;四分位数范围[IQR]: 59-76岁;59例男性),在症状出现后48分钟内(IQR, 27-93分钟)进行基线平扫CT扫描;基线MRI在中位数为38分钟(IQR, 24-48分钟)后进行扫描。在急性DW MRI扫描中,算法检测到的病灶体积与专家勾画的参考标准病灶体积具有相关性(r = 0.76, P < .001)。算法分割体积之间的平均差值(中位数,15 mL;IQR, 9-38 mL)和DW MRI容积(中位数,19 mL;IQR, 5-43 mL)为11 mL (P = .89)。
