Eur Radiol: 影响鼻胃管X线可见度不足的因素:一项回顾性分析

  鼻胃管植入术是一种常见的具有多种医疗用途的方法,医学治疗方法,例如处理胃肠道出血、胃减压、或当患者不愿或不能进食时的治疗管理(如药物、管饲)。鼻胃管插管对患者的安全有很大的风险,可造成严重的伤害甚至死亡。

    现阶段,胸部x线成像已被常规用于盲插鼻胃管的定位。高质量的x线成像可以增加管尖定位的明确可见性,从而避免由于置管位置不当(如肺插管、气胸、小肠插入)造成的风险。然而,患者的年龄、性别、体重、体高、体重指数、认知意识水平等患者相关因素以及导丝管材质、患者体位等环境因素均会影响x线的成像效果及质量,进一步影响鼻胃管的定位。


    近日,发表在European Radiology杂志的一项研究比较了不同条件下鼻胃管X线可见度的差别,进一步确定了影响成人鼻胃管x线可见性的患者相关因素及环境因素。

 

   对在一家综合医院进行的一项前瞻性随机试验中纳入的所有患者(N = 215)的常规收集的临床数据进行回顾性描述性分析。对每个需要鼻胃管置管的患者行胸部x光检查,然后由3名放射科医生独立评价鼻胃管在x光上的可见性和位置。

    所有患者中有14.9% (n = 32)的患者图像质量不足,无法确定鼻胃管定位。患者相关预测回归模型(性别、年龄、体重指数)解释了鼻胃管可见性不足的21%的方差(Nagelkerke R2 = 0.21)。环境因子回归模型表明,X射线摄片期间导丝是否在管内可以作为X射线鼻胃管明确可见性的预测因子。

    高体重指数、男性、摄片时鼻胃管内没有导丝与x线片上鼻胃管可见性不足的风险相关。在行胸片检查以确定鼻胃管的位置时,需要特别关注患者的基本信息,并适当调整方案以获得最佳的成像效果。
