使用定量聚合酶链反应(PCR)监测BCR-ABL1的分子反应(MR)是指导酪氨酸激酶抑制剂治疗和慢性髓系白血病(CML)患者长期随访的关键工具。对MR监测结果根据国际标准(IS)进行了标准化，定义最佳反应和治疗失败的具体时间依赖分子已被纳入治疗建议。在CML中使用外周血（PB）代替骨髓（BM）抽吸物监测MR的常规做法受到质疑。一些研究描述了成对的PB和BM标本中BCR-ABL1水平之间的差异。在此项回顾性单中心研究中，研究人员检测了283例CML患者的631对PB和BM样本，使用标准化定量逆转录(qRT)-PCR检测BCR-ABL1 IS。

研究结果显示，BCR-ABL1 IS结果总体上有较好的一致性，在主要MR的CML患者样本中，可以观察到PB中BCR-ABL1 IS水平系统性升高的趋势。这种差异在接受伊马替尼治疗至少1年的患者中最为明显。重要的是，与PB抽吸物相比，当在PB中评估BCR-ABL1IS时，差异导致深部MR率显著降低。

综上所述，数据表明，在CML患者中，PB对深部MR的分类比BM更为严格。研究人员的研究支持了目前主要使用铅对慢性粒细胞白血病进行长期分子随访监测的做法。
