         在生物制剂时代,内镜下病变的愈合已成为克罗恩病(CD)的治疗目标,因为实现此目标与改善长期临床疗效相关,包括更高的持续临床缓解率,住院需求的降低以及减少了手术需求。最近的研究探索使用磁共振小肠造影(MRE)作为一种用于检测与TNF-α抑制剂剂和/或皮质类固醇的治疗反应的工具,在CD患者中,对肿瘤坏死因子α(TNF-α)抑制剂治疗的反应有所不同,只有30%的患者会获得长期缓解,因此,本项研究的主要目的是确定与长期治疗成功相关的治疗前MRE的相关预测因素。

 

 

         这项前瞻性纵向单中心研究纳入了具有临床活动期的CD患者,这些患者需要用至少一种TNF-α抑制剂治疗,并通过MRE检测到严重的炎性病变(分段MaRIA≥11)。MRE数据是在基线以及第14和46周时获得的。主要观察终点是每个肠段中严重炎症病变的愈合(MaRIA <11)情况。次要终点是按患者分析所有严重炎症性病变的愈合情况。

 

 

        本项研究共纳入了58例患有严重炎性病变的CD患者。在第46周时,在51/86(59.3%)段发现了严重病变的愈合,而在28/58(48.6%)患者中发现了所有段中炎症病变的完全愈合。多变量分析发现,严重炎症长期治愈的基线阴性预测指标是回肠位置(相对于结肠)(OR 0.00,[0.00-0.56] P = 0.002)和MRE上存在匍行脂肪(OR 0.00 [0.00-0.00] 0.57];P = 0.001)。在第14周时,MaRIA节段评分的持续性> 10.6是第46周愈合的阴性预测指标(OR 0.3 [0.04-0.38];P<0.001)。
