Radiology:如何识别腹主动脉瘤快速生长速率?

腹主动脉瘤(AAAs)合并腔内血栓(ILT)是预测动脉瘤生长的指标。

背景

腹主动脉瘤(AAAs)合并腔内血栓(ILT)是预测动脉瘤生长的指标。

目的

本研究旨在利用CT和MRI评估ILT在AAA进展中的价值。

材料与方法

本回顾性研究纳入了AAA且行对比增强CT剂随访时黑血MRI的患者。在多平面重建下测量AAA最大径,计算动脉瘤生长率。利用单变量和多变量线性回归模型评价人口学特征和影像学指标与动脉瘤生长的关系。

结果

本研究功能纳入了22例患者。共有207例行CT随访,18例患者行MRI随访。基线时AAA中位数长径为3.8 cm (IQR, 3.3-4.3 cm); 共有127/ 225 例(54.7%)出现ILT。与无ILT的AAA患者相比,合并有ILT的AAA的基线动脉瘤长径较大(中位数, 4.1 cm [IQR, 3.6-4.8 cm] vs 3.4 cm [IQR, 3.2-3.9 cm]; P < .001),具有较快的生长率(中位数, 2.0 mm/y [IQR, 1.3-3.2 mm/y] vs 1.0 mm/y [IQR, 0.4-1.8 mm/y]; P < .001)。合并ILT的小AAA(大小范围为3-4 cm)的生长速度是无ILT的生长速率快1.9倍(中位数, 1.5 mm/y [IQR, 0.9-2.7 mm/y] vs 0.8 mm/y [IQR, 0.3-1.5 mm/y]; P < .001)。合并ILT的中度大小AAA的生长速率是无ILT的生长速率的1.2倍(中位数生长速率, 2.1 mm/y [IQR, 1.4, 3.7 mm/y] vs 1.8 mm/y [IQR, 0.9, 2.0 mm/y]; P = .06)。在多元分析中,基线长径和ILT是与动脉瘤生长速率独立正相关的指标(标准回归系数分别为0.43 [P < .001] and 0.15 [P = .02])。

结论

最大横截面长径与腔内血栓是预测腹主动脉瘤生长的独立指标。
