2015年美国胸科协会主席在首次世界肺功能日(6月27日)呼唤:像量血压一样检查肺功能。钟南山像量血压和测血糖对控制高血压和高血糖的重要性一样,肺量计(肺通气功能检查)应作为肺部疾病筛查和检测的一部分。

什么是肺功能检查呢?

肺功能检查是指采用一系列手段检测肺的通气功能、弥散功能和气体交换功能的人体的生理和病理改变的一种检查方法。从而判断肺的生理和病理改变,这种检查方法称为肺功能检查。目的是了解呼吸系统的生理状态,明确肺功能障碍的类型,判定病变损害的程度,评估肺的功能储备。肺功能检查是一种无创的检查,主要是查看肺部的通气功能和肺部的换气功能,从而判断有没有肺部疾病。医生可以根据具体的检查结果查出有没有肺部的病变以及气道的病变情况。

什么人需要做肺功能检查?

钟南山院士呼吁下列社区面对以下四类人应推广肺功能普查:1.大于40岁者;2.吸烟人群;3.职业气体污染人群;4长期重度空气污染地区的人群。

慢性阻塞性肺疾病(COPD)、慢性支气管炎、肺气肿等呼吸道疾病患者定期复查——监控病程发展

季节性咳喘发作——看是否患有哮喘

有慢性咳嗽、呼吸困难、气促、喘息、胸闷等表现的病人——明确原因

反复上呼吸道感染者——观察肺功能是否有损伤

吸烟并长期咳嗽,或长期大量吸烟者——看小气道功能是否改变

胸片异常——判断肺功能损害程度

麻醉、外科手术前——评估手术风险,以及术后恢复的预测

呼吸疾病临床治疗后的疗效评估和疾病进展评估

检查时我们应该怎么配合?

检查过程一定要患者密切配合呼气吸气动作,配合度越高,准确率越高

,请您按如下要求进行准备:

检查前安静休息15分钟

练习用口深吸气后,再快速用力(爆发力)吹气并持续6秒不中断的动作(如:吹蜡烛)

需按医嘱要求停用相应支气管扩张剂,如茶碱类、b2受体激动剂、激素类,抗过敏药等,如不明白请向医生咨询

检查当天可以如常进食早餐

肺功能检查对我有没有伤害?

肺功能检查是一种物理检查方法,对人体无任何损伤,无痛苦和不适;但有些项目偶尔会引起一些不适,如咳嗽、胸闷、气紧、喘鸣、心悸、轻微手颤、声嘶、咽痛、头晕、头痛、面红等,一般可经药物治疗或休息后自行缓解。部分病人检查前需注意排除不适宜检查的情况。请咨询您的医生。

肺功能检查有什么意义呢?

肺功能检查可以

n 早期检出肺、呼吸道病变,如慢性支气管炎、肺气肿、支气管哮喘、间质性肺病等。

n 鉴别呼吸困难的原因,是哮喘还是阻塞性肺病等导致的,还可以判断气道堵塞的位置。

n 评估肺部疾病的病情严重程度。

n 评估外科手术(特别是胸部手术)耐受力及术后发生并发症的可能性。

n 长期吸烟的人也应定期做肺功能检查,以观察肺功能受损的情况,督促患者下决心戒烟。

n 危重病人的监护等。

n 有助于明确COPD的严重程度,并依据疾病严重程度制定相应的治疗方案。

肺功能具有敏感度高,重复检测方便等优点,与胸片、胸部CT等影像学检查注重外形结构改变相比,肺功能检查更侧重于了解支气管、肺部的功能性变化如小气道气流受限、通气功能、弥散功能等。

哪些病人不适宜做肺功能检查?

近周内有大咯血、气胸、巨大肺大泡、心功能不稳定者;

支气管扩张剂过敏者;

喉头或声带水肿、中度或以上通气功能异常者不宜进行支气管激发试验。

我院肺功能室是呼吸内科属下的专业临床检查科室,是桂西地区最早开展肺功能检查的科室,现有德国耶格公司生产的电脑化肺功能仪Master Scope等多台套先进的诊疗设备,每年检查约2万人次。我科不仅有先进的肺功能检测仪器,更有一支专业业务强大的技术力量,现有主任医师4名、副主任医师6名、主治医师5名、博士3人、硕士7人、专职主管护师3名。目前本科室已能进行国际上已开展的绝大多数肺功能检测项目,包括分段肺容量、肺通气功能、弥散功能、体积描计、气道阻力、脉冲振荡呼吸阻抗、一氧化氮呼出气测定、支气管舒张试验、支气管激发试验、运动激发试验、鼻激发试验、能量代谢、运动心肺功能及运动肺康复等。
