Eur Respir J:中国人群大规模全基因组关联分析发现肺功能的新位点

肺功能是一种可遗传的复杂表型,肥胖是其重要危险因素之一。然而,关于两者共同遗传基础的相关知识仍然有限。大多数肺功能的全基因组关联分析研究(GWASs)是基于欧洲人群,缺乏其他人群的大规模肺功能全基因组关联分析。

发表在Eur Respir J杂志的一篇文章使用中国慢性病前瞻性研究(CKB)和英国生物样本库(UKB)数据进行了大规模GWAS和跨性状/群体分析,以1)识别中国人群肺功能性状的新遗传风险位点,包括FEV1、FVC和FEV1/FVC;2) 研究中国和欧洲人群肺功能特征与肥胖特征(体重指数(BMI)、BMI校正的腰臀比和BMI校正腰围(WCadjBMI))之间的共同遗传效应;3)通过CKB和UKB队列,调查基线BMI和BMI的纵向变化是否会影响肺功能,同时考虑肺功能的多基因性。

研究纳入CKB的100285名受试者。为识别肺功能的新基因位点,研究人员对CKB患者FEV1、FVC和FEV1/FVC进行了单性状GWAS分析。随后,对肺功能和肥胖特征进行全基因组跨性状分析。最后,在CKB中开发了肺功能多基因风险评分(PRSs),并研究其与BMI相关性对肺功能的相互作用。研究人员还使用英国生物样本库(UKB)的457756例受试者与CKB平行进行跨性状分析,以研究群体特异性效应。

研究人员在CKB中发现了9个FEV1、6个FVC和3个FEV1/FVC全基因组新位点。在CKB和UKB中,FEV1和FVC均与肥胖性状呈显著负相关(Rg=−0.26, p=5.99×10−9for FEV1–WCadjBMI; Rg=−0.11, p=5.44×10−3for FVC–BMI; Rg=−0.28, p=2.24×10−10for FVC–WCadjBMI)。肺功能和肥胖性状之间共享的基因位点突出了重要的生物学途径,包括细胞增殖、胚胎、骨骼和组织发育以及基因表达调控。孟德尔随机分析表明,在CKB和UKB中,BMI对FEV1和FVC都有显著的负因果效应。在平均8年随访期间,肺功能PRSs显著改变了BMI变化对肺功能的影响。

这项大规模的肺功能GWAS确定了肺功能和肥胖之间的新基因位点和共同的遗传病因学。根据受试者的多基因背景,BMI变化可能对肺功能产生不同的影响。这些发现可能为开发改善肥胖和肺功能的分子靶向治疗开辟新途径。
