Nature:哈佛大学医学院马秋富研究团队揭示针灸背后的生物学原理

来自哈佛大学医学院马秋富教授团队的研究人员探究了为什么电针刺激(electroacupuncture stimulation,ES)能从后肢足三里(ST36)穴驱动最近报道的迷走-肾上腺轴。

针灸治疗人类疾病的一个核心思想是,对身体特定区域(穴位)的刺激可以调节远处的身体生理机能,这种效果被认为是通过假设的经络通道来实现的。虽然现代解剖学研究还没有支持这种通道的实际存在,但长距离的针灸效果可以通过体感自律反射来实现。这些反射首先是由位于背根神经节(DRG)和/或三叉神经节的神经元激活外周神经,然后感觉信息从脊髓传输到大脑,随后激活外周自主神经通路,最终调节身体生理的各个方面。

在最近的一项研究中,来自哈佛大学医学院马秋富教授团队的研究人员探究了为什么电针刺激(electroacupuncture stimulationES)能从后肢足三里(ST36)穴驱动最近报道的迷走-肾上腺轴,而不是从腹部天枢(ST25)穴驱动,在天枢穴,ES反而能驱动脊柱交感神经反射。

研究人员作出推测,躯体-迷走-肾上腺反射是由支配后肢所特有的组织,如关节、骨骼和骨骼肌,的感觉通路驱动的。一个候选途径包括由PROKR2Cre标记的DRG神经元(其中用Cre介导的重组标记具有持续或短暂表达prokineticin受体2的神经元)。这些神经元支配肢体深层组织,但不支配皮肤表皮。

PROKR2ADV纤维的激活驱动迷走神经-肾上腺抗炎轴。

实验数据表明,PROKR2Cre标记的感觉神经元,其支配后肢深层筋膜(如骨膜)而不是腹部筋膜(如腹膜),对驱动迷走神经-肾上腺轴至关重要。在ST36部位的低强度ES在小鼠的PROKR2Cre标记的感觉神经元中未能激活后脑迷走神经元或驱动肾上腺的儿茶酚胺释放。因此,ES不再抑制细菌内毒素诱发的系统性炎症。相比之下,ST25和ST36部位的高强度ES诱发的脊髓交感神经反射没有受到影响。

研究人员还表明,通过ST36位点对PROKR2Cre标记的神经末梢进行光遗传刺激,足以驱动迷走-肾上腺轴,而不是交感神经反射。此外,PROKR2Cre神经纤维的分布模式可以回顾性地预测低强度ES会或不会有效产生抗炎作用的身体区域

因此,该研究为穴位驱动特定自律神经通路的选择性和特异性提供了一个神经解剖学基础。
