较高的体重指数(BMI)可降低重度细菌感染导致的死亡。

肥胖是一个迅速增长的全球健康问题,对预期寿命和健康经济学有很大影响。与正常体重的人群相比,肥胖的人更容易患多种形式的癌症和血管疾病。尽管解释肥胖如何导致不同疾病的机制尚未完全阐明,但此前已有证据表明肥胖会影响免疫系统。肥胖被认为与免疫细胞功能紊乱、慢性低度炎症有关。但是现在一项研究表明,较高的体重指数(BMI)可降低重度细菌感染导致的死亡。

近日,瑞典哥德堡大学Skaraborg医院的科学家们进行了这项研究,并将结果发表在PLOS ONE杂志上。

该研究在9个月的时间里对所有2196名因怀疑严重细菌感染而住院接受治疗的成年人进行观察。研究人员在住院期间和住院后对该研究人群中的病人进行了长期跟踪。

结果显示,在短期和长期内,即住院后28天和一年内,生存机会的提高与较高的体重指数(BMI)有关,存活率的差异明显。

在正常体重组中,26%的人在一年内死亡。在体重指数较高的群体中,9-17%的人在一年内死亡。

事实上,此前已有研究对有限的病人群体进行的不定期调查显示类似的结果。而此次研究发现澄清并证实了 "肥胖生存悖论"----超重和肥胖提供了对严重细菌感染的保护。

该研究的第一作者Sahlgrenska学院传染病高级讲师Åsa Alsiö教授表示,超重和肥胖是癌症、心血管疾病等许多疾病的致病因素,特别是COVID-19,其中较高的BMI与较高的死亡率有关。矛盾的是,这里的情况恰恰相反。

Alsiö指出,目前还不知道高BMI如何有利于细菌感染的病人,或者是否与免疫系统的功能以及调节有关。认为有必要在人群层面上进行更多的研究,了解体重指数如何影响各种传染病的治疗效果,以及与免疫系统的调节可能存在哪些联系。
