阿司匹林所致的痴呆副作用,其机制是什么?

阿司匹林作为一种百年老药自发现以来,已为人类生命做出了诸多贡献,给人们生活带来了重大改变。除了常规的消炎镇痛作用,它还被广泛地用于心血管疾病的预防。

任何药物都有一定的副作用,阿司匹林也不例外,比如消化道出血、痴呆和认知障碍,前者主要跟其刺激胃肠道黏膜有关,后者的作用机制尚不清楚。

为此,有学者进行了探索,旨在ASCEND研究中进行预先指定的二次分析(随访时间更长),来评估低剂量阿司匹林对痴呆和认知障碍风险的影响,并在AHA大会上进行了分享。

 

研究方法

 

该研究纳入了2005年6月-2011年7月期间在ASCEND试验中的15427例无心血管疾病史且无痴呆记录的英国糖尿病患者。随机分为两组:阿司匹林组(10mg/d)及安慰剂组,平均随访时间为7.4年。通过医务人员报告或患者的住院诊断确定痴呆(广义的)结局:痴呆、认知障碍或意识模糊。

此外,除了比较阿司匹林对广义痴呆结局的影响外,在校正年龄、既往疾病和基线风险因素后,研究者还评估了阿司匹林与严重血管事件和大出血及随后患痴呆风险的相关性。

 

研究结果

1. 在治疗期(平均随访7.4年)及之后平均随访1.7年内,共有1146名受试者发生痴呆。

2. 发生非致命性严重血管时间(n=990)和大出血(n=496)与广泛痴呆结局的风险比分别为2.39(95%CI 1.96-2.92)和1.99(1.52-2.60)。(揭盲后分析结果将另外公布)。

 

综上所述,ASCEND研究将为阿司匹林对糖尿病患者患痴呆症的影响提供额外的证据,期待更多的试验结果公布。
