Int J Cardiol:长期接触细颗粒物与房颤

虽然许多研究已将空气污染中细颗粒物(PM2.5)的升高与不良血管结果联系起来,但PM2.5长期暴露对一般人群中与空气污染相关的事件性心房颤动(AF)尚未得到很好的研究。

近日,血管领域权威杂志International Journal of Cardiology上发表了一篇研究文章,研究人员纳入了2009年至2013年期间来自韩国国家健康保险局-国家样本队列中未被诊断为AF的432587名受试者。从2002年1月开始筛查医疗记录,以调查受试者的无病基线期。这些受试者随访至2013年12月。在研究期间研究人员将受试者的住宅邮政编码与每小时测量的空气污染物(微粒和气体)浓度和气象(温度和湿度)数据进行匹配。

在1666528人次每年随访期间,5825名受试者(350人/100000人每年)观察到事件性AF。研究人员发现事件性AF与PM2.5(增加10μg/m3的HR=1.179 [1.176-1.183],p<0.001)、PM10(增加10μg/m3的HR=1.034 [1.033-1.036])和气态空气污染物的长期平均浓度之间存在显著的关联。当将受试者分为亚组时,这些长期暴露于PM2.5的效应在男性(HR=1.187 [1.183-1.192],p<0.001)、年龄较大的受试者(年龄≥60岁; HR=1.194 [1.188-1.200],p<0.001)、肥胖者(体重指数≥27.5kg/m2,HR=1.191 [1.183-1.199],p<0.001)、既往有心肌梗死的受试者(HR=1.203 [1.186-1.221]) ,p<0.001)和高血压病史者(HR=1.191 [1.185-1.197],p<0.001)中更为明显(与相反的亚组相比,每个因素相互作用p<0.05)。

由此可见,即使在亚洲普通人群中,长期暴露于PM2.5与AF的发病率增加有关。在60岁以上且有高血压或既往心肌梗死病史的肥胖男性中更为明显。
