Heart：流感疫苗与急性心肌梗死风险

接种流感疫苗对普通人群AMI的中等保护作用一致，主要在于一级预防。

近日，心脏病领域权威杂志Heart上发表了一篇研究文章，该研究旨在评估不同流行时期流感疫苗接种与普通人群首次急性心肌梗死(AMI)风险之间的关系。

这是一项基于40-99岁患者人群进行的病例对照研究。对于每个AMI病例，随机选择五个对照者，分别匹配准确的年龄、性别和日期（AMI诊断）。如果患者在同一季节内索引日期之前至少14天记录了流感疫苗接种，则认为已接种疫苗。

研究人员通过条件Logistic回归、计算调整后的OR(AOR)及其各自的95%CI为来评估流感疫苗接种与AMI风险之间的关联。

研究人员确定了24155名AMI病例和120775名匹配的对照者。其中，分别有31.4%和31.2%的个体接种了疫苗，AOR为0.85（95%CI为0.82至0.88）。没有观察到性别、年龄和背景心血管风险的影响。接种疫苗后不久观察到AMI风险降低，并随着时间的推移持续存在。在流行前(AOR=0.87;95%CI为0.79至0.95)、流行期(AOR=0.89;95%CI为0.82至0.96)和流行后(AOR=0.83;95%CI为0.79至CI0.79)期间获得了类似的结果。未发现与肺炎球菌疫苗之间存在相关（AOR=1.10；95%CI为1.06至1.15）。

由此可见，接种流感疫苗对普通人群AMI的中等保护作用一致，主要在于一级预防。
