JAHA：主动脉疾病患者颅内动脉瘤患病率如何？

主动脉疾病(AD)患者可能具有较高的颅内动脉瘤(IA)患病率。在当前以人群为基础的研究中，AD患者中IA的患病率是普通人群的四倍。对于AD患者，可以考虑进行早期IA筛查以及时得到治疗。

主动脉疾病(AD)患者可能具有较高的颅内动脉瘤(IA)患病率。

近日，心血管疾病领域权威杂志JAHA上发表了一篇研究文章，该研究旨在利用全国代表性队列样本数据评估了AD患者IA的患病率，并确定了IA的潜在危险因素。

研究人员把AD定义为主动脉夹层和主动脉瘤，该研究使用了来自韩国国民健康保险服务全国样本队列数据库中110万名患者作为全国性代表队列样本，并采用χ2检验或Fisher精确检验分析了AD患者中IA的患病率及其合并的潜在危险因素。

AD患者IA的患病率为6.8%(155/2285)，AD患者合并IA的调整后比值比(OR)为3.809 (95%CI为3.191-4.546；P<0.01)。伴有高血压的AD患者发生IA的可能性是正常人的19倍(调整后的OR为18.679；95%置信区间为16.555-21.076；P<0.01)。伴有糖尿病、老年(>60岁)和男性AD患者发生IA的可能性分别升高4倍、3倍和2倍(调整后OR分别为4.291、3.469和1.983；95%CI分别为3.914~4.704、3.152~3.878和1.779~2.112)。社会经济地位或残疾的亚组分析发现IA的患病率在各组患者中均显著升高。

由此可见，在当前以人群为基础的研究中，AD患者中IA的患病率是普通人群的四倍。对于AD患者，可以考虑进行早期IA筛查以及时得到治疗。
