钠-葡萄糖共转运体2抑制剂可改善射血分数降低的心力衰竭患者的预后,但是关于血糖水平高低是否影响其对心力衰竭和肾脏事件的益处,还需要更多的信息。 EMPEROR-Reduced试验招募了射血分数≤40%的II-IV级心衰患者,随机分至两组,在推荐治疗之外还接受恩格列净(10 mg/日)或安慰剂治疗。本文报道了恩格列净对有或无糖尿病的患者的心血管及肾脏预后的影响。 恩格列净对主要终点的效益:A 合并糖尿病;B 无糖尿病 共招募了3730位患者,其中1856位(50%)合并糖尿病,1268位(34%)为糖尿病前期(血红蛋白A1c[HbA1c] 5.7-6.4%),606位(16%)的血糖正常(HbA1c<5.7%)。合并糖尿病的患者的主要预后(心血管死亡或心衰住院)、总心衰住院量和肾脏不良预后的风险均更高,而糖尿病前期和血糖正常患者的这些风险相近。 恩格列净对心衰住院的效益:A 合并糖尿病;B 无糖尿病 恩格列净可同时降低合并糖尿病和不合并糖尿病的主要预后风险(风险比[HR]分别是0.72和0.78)。在总心衰住院、估计肾小球滤过率减退和严重肾脏不良预后等方面上,有无合并糖尿病的患者从
