全清醒局部麻醉下的单指再植风险更小,费用更低

手指再植已经成为一种常见的外科手术。默认情况下在全身麻醉或臂丛神经阻滞下进行。

而全清醒局部麻醉下的断指再植是一种具有挑战性的方法,关于这一过程的文献也很少。

本文介绍了术者在完全清醒局部麻醉下与全身麻醉下断指再植的临床经验。

51例患者在首次锐器截肢后接受单指再植,其中16例接受完全清醒局部麻醉,35例接受全身麻醉治疗。完全清醒局部麻醉的适应症为锐利截肢损伤、估计手术时间少于3小时以及患者的配合。使用1%或2%利多卡因在患指掌指关节掌侧中点浸润进行全清醒局部麻醉,无需镇静药物。人口统计学数据包括手术结果、等待时间、手术时间和住院时间。

该研究共纳入51名连续患者。结果显示,完全清醒局部麻醉组的等待时间和手术时间明显缩短,其他参数无显著差异。总成功率为76.47%,平均总手术时间为207分钟。

综上述,术者介绍了在完全清醒局部麻醉下进行手指再植手术的临床经验,其成功率为81.25%,与全身麻醉相比无显著差异。

术者建议在某些适应症下,在完全清醒的局部麻醉下进行断指再植,其中包括单指截肢、无严重挤压伤,估计手术时间不到3小时,以及一名心理稳定、合作的患者,没有相关的危及生命的损伤。

在选定的患者中,在完全清醒的局部麻醉下,断指再植可以成功完成,与全身麻醉相比,具有更低的麻醉风险和更少的医疗费用。该方法对单指再植是可行的。因此,在完全清醒的局部麻醉下断指再植是一种可行的替代全身麻醉的方法。
