Dig Dis Sci:超氧化物歧化酶水平可预测急性胰腺炎早期的持续循环衰竭和死亡风险

氧化应激是急性胰腺炎(AP)发病机制中的重要组成部分。超氧化物歧化酶是体内主要的抗氧化酶。这项研究的目的是调查急性胰腺炎发作初期超氧化物歧化酶活性的变化及其在预测器官衰竭和死亡风险中的价值

背景及目的

氧化应激是急性胰腺炎(AP)发病机制中的重要组成部分。超氧化物歧化酶是体内主要的抗氧化酶。这项研究的目的是调查急性胰腺炎发作初期超氧化物歧化酶活性的变化及其在预测器官衰竭和死亡风险中的价值。

 

 

方法

研究人员从前瞻性数据库中提取了2013年至2017年住院的2549例患者的数据,在其中选择了854名在发病24小时内入院的成年患者,并收集了完整数据。研究人员比较了不同严重程度,器官衰竭和死亡率的患者入院第一天,第二天和第三天的血清超氧化物歧化酶活性。曲线下的面积用于预测器官衰竭,胰腺坏死和死亡率,使用ROC曲线进行估算。

 

 

结果

在854名成年患者中,轻度急性胰腺炎,中度重度急性胰腺炎和重度急性胰腺炎患者的超氧化物歧化酶活性显着不同(P  = 0.005)。持续性肾衰竭(77.8±37.2),持续性循环衰竭(66.2±14.9)和死亡率(64.3±16.0)的患者中超氧化物歧化酶活性显着降低。超氧化物歧化酶在预测持续性循环衰竭和死亡率方面的准确性很高,ROC曲线下的面积分别为0.83和0.84。

 

 

结论

超氧化物歧化酶活性与AP的严重程度和临床结果呈负相关。超氧化物歧化酶活性在预测AP早期的持续性循环衰竭和死亡率方面非常准确。
