ABCA4转运蛋白的功能障碍导致有毒的类视黄醇积聚,从而使视网膜色素上皮和感光细胞变性。视力丧失通常在儿童时期出现并发展。通过ABCA4测序确诊。

患者为一名13岁男孩,因双眼中心视力盲点扩大而到眼科门诊就诊。他没有眼疾家族史。

就诊时,右眼视力为20/40,左眼视力为20/50。两只眼睛眼底检查发现广泛的、黄色、鱼形斑点。短波自发荧光成像检查发现弥散性超自发荧光病变,提示双视黄醛积聚和视网膜色素上皮中央部萎缩,伴有乳头周围变薄(如图A所示)。这些发现与诊断青少年黄斑变性(stargardt氏病)相一致,后者是由ATP结合盒亚家族A成员4(ABCA4)编码基因的突变引起的常染色体隐性遗传疾病。

ABCA4转运蛋白的功能障碍导致有毒的类视黄醇积聚,从而使视网膜色素上皮和感光细胞变性。视力丧失通常在儿童时期出现并发展。通过ABCA4测序确诊。建议患者避免补充维生素A,因为维生素A可能会加速类维生素A的积累。

在6年的随访中,右眼的视力为20/70,左眼的视力为20/50,中心盲点恶化。自发荧光成像发现先前确定的鱼鳞状斑点区域渐进性萎缩,仍伴有视乳头周围变薄(如图B所示)。
