Euro Radio:利用CT纹理分析来发现肝细胞肝癌TACE治疗后的反应!

研究基于肿瘤大小和治疗前CT纹理参数的嵌套多参数决策树模型准确预测肝癌对经导管动脉化疗栓塞术(TACE)反应的可行性。

目的:研究基于肿瘤大小和治疗前CT纹理参数的嵌套多参数决策树模型准确预测肝癌对经导管动脉化疗栓塞术(TACE)反应的可行性。

材料与方法:本项研究为回顾性研究(2011年1月至2017年9月),包括37位患者一共92个肝细胞肝癌的病灶,每个病灶都经过穿刺活检病理证实为肝细胞肝癌,均有治疗前和治疗后连续的动态CT扫描图像,每个病灶都经过TACE治疗。 根据修改过后的实体肿瘤的反映评价标准,在基线动脉期CT图像上对病变进行手动分割以提取肿瘤大小和定量纹理参数。肝细胞肝癌被分为原发性TACE(VT-病变)及反复性TACE(RT-病变)的病变。 生成不同的多参数决策树模型用于预测完全缓解(CR)和进展病灶(PD)两组病灶。同时评价了AUC和模型准确性。

结果:72例VT病变和20例RT病变中各有38例(52.8%)及8例(40%)病变达到了CR标准。 尽管进行了TACE治疗,但在随访影像学检查中仍有16例VT病变(22.2%)和8例RT病变(40%)表现为PD。 VT病变的阳性像素平均值(MPP)显著高于RT病变(180.5 vs 92.8,p = 0.001)。 在ROC曲线分析和准确性方面,预测VT病变达到CR标准时得到的最高的AUC为0.96,阳性预测值为96.9%,准确性为88.9%,此结果均高于在预测VT病变中表现为PD(AUC 0.88,准确度80.6%)、RT病变中达到CR标准(AUC 0.83,准确度75.0%)以及RT病变中表现为PD(AUC 0.86,准确度80.0%)这三种情况。

结论:以肿瘤的异质性和尺寸为基础的嵌套多参数决策树模型可以非常准确的预测肝细胞肝癌病灶对TACE治疗对反映。嵌套多参数决策树模型可以用作多学科治疗决策过程中的附加标准。
