    骨骼肌磁共振成像(MRI)是评估神经肌肉疾病中的肌肉受累情况的临床常用的无创影像学技术。MRI评估依赖于常规序列,包括T1加权和脂肪抑制T2加权图像。最近,新开发的可量化肌肉内脂肪分数的肌肉MRI技术已成为显示各种肌肉病变病理情况的潜在生物标志物。

    腓骨肌萎缩症 (CMT)是最常见的遗传性多神经病变,其特征是隐匿性起病,主要表现为远端肌肉萎缩、运动和感觉丧失。虽然目前除了物理或职业治疗外,还没有治愈这种疾病的方法,但一些使用治疗药物的临床前和临床研究显示了令人鼓舞的结果。因此,使用无创方法进行早期诊断或疾病监测对这些患者的治疗意义重大。


    近日,发表在European Radiology杂志的一项研究探讨了T1加权图像的纹理特征与定量脂肪成像获得的脂肪分数的相关性,同时通过比较健康志愿者和CMT患者的纹理特征来评估其检测病变的潜力。

    探讨使用T1加权图像的纹理特征是否与脂肪分数相关,以及它们在腓骨肌萎缩症 (CMT) 患者和健康志愿者之间是否存在差异。

    本项回顾性研究得到了机构审查委员会的批准,并放弃了知情同意的要求。本研究使用了来自先前研究的18名CMT患者和18名健康志愿者的数据。肌肉的纹理特征包括来自T1加权图像信号强度的平均值、标准差(SD)、偏度、峰度和熵。采用Spearman相关分析评价纹理特征与基于三维多梯度回波dixon序列测量的脂肪分数之间的关系。采用Mann-Whitney U检验比较CMT患者与志愿者的纹理特征。使用类内相关系数评价观察者内部和观察者之间评价纹理特征的一致性。

    本研究表明使用T1加权图像的纹理特征与脂肪分数具有相关性,表明这些特征有潜力量化肌肉内脂肪浸润的程度,且在CMT患者和健康志愿者之间存在差异,为临床提供了一个早期诊断和疾病监测的无创的影像学方法。
