在检测心脏瓣膜钙化的常规影像学检查中有机会识别罹患外周动脉疾病（PAD）风险增加的个体。近日，心血管领域权威杂志Eur Heart J-Card Img上发表了一篇研究文章，研究人员调查了主动脉瓣钙化（AVC）和二尖瓣环钙化（MAC）与发展为临床PAD风险或踝肱指数低（ABI）之间的关系。

研究人员评估了6778名2000至2002年期间无PAD的动脉粥样硬化多民族研究参与者心脏CT上AVC和MAC。临床PAD评估直至2015年。研究人员评估了5762名基线ABI>0.9的个体的低ABI事件，即ABI<0.9和下降≥0.15，这些参与者在3-10年后至少进行一次ABI随访。调整后的Cox比例风险和Poisson回归模型分别用于确定瓣膜钙化与临床PAD和低ABI之间的关系。

中位随访14年年和9.2年，分别有117例临床PAD和198例低ABI事件发生。MAC的存在与临床PAD风险增加相关[风险比为1.79；95％置信区间（CI）为1.04–3.05]，但ABI值不低（比值比为1.28；95％CI为0.75–2.19）。没有观察到AVC的存在与任何一种临床PAD的风险之间存在显著相关性。

由此可见，MAC与发展为临床PAD的风险增加相关。需要进一步的研究来证实该研究的发现，并更好地了解MAC作为PAD的风险标志，是否具有任何预测价值。
