JCEM：老年糖尿病患者肥胖与心血管疾病之间的关系

虽然BMI不足以作为肥胖相关结局的风险指标，但WC是合适的替代指标。

一些研究表明，伴有肥胖的2型糖尿病(T2DM)患者比正常体重的T2DM患者具有更好的临床结局。

近日，内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章，研究人员旨在评估无CVD的老年T2DM患者肥胖、心血管疾病(CVD)事件和死亡率之间的关联。

该研究为2009-2017年期间进行的回顾性观察研究，平均随访7.26年。本研究使用韩国国家健康信息数据库纳入了249903名患有T2DM且既往无CVD的老年（≥65岁）患者。

研究人员根据体重指数(BMI)和腰围(WC)对受试者进行分类，并使用Cox比例风险回归分析了卒中、心肌梗死和全因死亡的复合事件，并调整了基线协变量。

该研究的复合主要结局的发生率为30.95人/1000人每年。主要结局风险分别与BMI和WC呈L形和U形关联。在多变量Cox比例风险模型中，最高BMI组（≥30kg/m2；风险比[HR]=0.824，95%置信区间[CI]：0.794−0.855）的主要复合结局风险低于正常BMI组（≥18.5和<23kg/m2）。相反，最高WC组（≥100cm/≥95cm；男性/女性；HR=1.434，95%CI为1.384-1.486）高于正常WC组（<90cm/<85cm；男性/女性）。

由此可见，虽然BMI不足以作为肥胖相关结局的风险指标，但WC是合适的替代指标。
