肢体周围神经损害在老年人中很常见,并且与身体限制和死亡风险增加有关。多达50%的老年人感觉周围神经功能(如触觉)可能受损。初步证据表明,周围神经损害在有认知障碍的人中也更常见。周围神经损害和痴呆症共享危险因素,包括年龄、糖尿病和血管疾病。尽管如此,很少有研究严格评估感觉或运动周围神经功能是否与偶发痴呆症有关。

确定周围神经损害与痴呆症之间的联系可以为痴呆症或周围神经病患者的临床护理和管理提供信息,并有助于识别健康结局特别差的高风险患者。这项工作可以帮助揭示外周和中枢神经退化的共同机制。许多慢性疾病会影响周围神经功能,包括糖尿病、血管疾病、维生素B12缺乏和肾脏疾病。周围神经功能不良也可能导致身体功能和行动不便,与慢性健康状况无关。这些并发症和后果通常也与痴呆有关。

一项小型研究表明,糖尿病背景下的周围神经病变与痴呆症风险无关,然而,在普通老年人口和其他共病亚群中,关于周围神经损伤与痴呆风险之间的联系的数据有限。

近日,研究人员调查了黑人和白人老年人的下肢周围神经损伤与发展为痴呆症的风险之间的关联。还进一步检查了3个基于病理生理学的不同危险因素之间的相关性是否存在差异:糖尿病并存状态、低维生素B12和载脂蛋白Eε4等位基因。

纳入研究的黑人和白人的健康、老龄化和身体成分研究参与者,年龄在70-79岁之间,登记时均无痴呆症。在第4年(本研究的分析基线)测量下肢感觉和运动周围神经功能。用单纤维(1.4g、10g)和足趾振动阈值测量感觉神经损伤。单纤维不敏感定义为无法检测到单纤维(3/4触摸),振动检测损害定义为>130µm。腓骨运动损害定义为<1毫伏(MV)复合运动动作电位(CMAP)波幅和神经传导速度减慢<40m/s(m/s)。随后11年的痴呆症事件是根据医疗记录、认知分数和药物来确定的。根据人口统计学和健康状况调整的Cox比例风险模型评估了神经损伤与偶发痴呆症的关联。

  • 在2174名受试者中(女性占52%,黑人占35%),45%的受试者无法检测到1.4g的单纤维,9%的受试者无法检测到10g的单纤维,6%的受试者感觉不到振动,10%的受试者CMAP波幅低,24%的受试者传导速度慢。
  • 单纤维10g(HR:1.35,95%CI:0.99,1.84)和振动检测不灵敏(HR:1.73,95%CI:1.24,2.40)与痴呆的风险相关。
  • 对单纤维1.4g、CMAP幅度和传导速度差异无统计学意义(P>0.05)。
  • 周围神经损伤的数量越多,痴呆的风险越高;≥3处周围神经损伤的HR=2.37(95%CI:1.29,4.38)。
  • 在分组分析中,除振动检测外,糖尿病、低维生素B12和载脂蛋白Eε4等位基因患者的效应估计值普遍较高。

周围神经损伤,特别是感觉损伤,即使在调整了年龄和其他健康因素后,也与痴呆症的更高风险有关。这些关联可能代表了神经系统退化的共同易感性。
